Health History Cultural Awareness Questions

This checklist consists of questions that will strengthen the healthcare provider's understanding of the patient's health history.   

• Why did you come to this facility today?

• Have you taken or done anything before this visit to treat your present condition?

• Do you use herbal treatments, home remedies or supplements?

• Have you ever sought medical treatment elsewhere?  Describe the nature of the previous experience.

• In the past, have you had any problematic experiences with the delivery of medical treatment?

• Have you experienced any between previous treatments and your current treatment.
